Key Candidate Requirements
- Licensure: Must be a PA licensed in North Carolina
- Experience: Minimum prior clinical experience required; fresh graduates not accepted
- Certifications: Current BLS/ACLS required
- Availability: Full-time, participation in on-call rotation
- Visa Status: PERM only; no locums or visa candidates
Clinical Role
- Collaborate with PACE physicians to deliver high-quality primary care to geriatric patients
- Perform initial, semi-annual, annual, and as-needed participant assessments
- Develop individualized care plans within the Interdisciplinary Team (IDT) framework
- Manage acute illnesses and chronic conditions
- Provide preventive care (screenings, immunizations, monitoring)
- Prescribe medications and therapies appropriately
- Refer patients to specialists when needed
- Participate in discharge planning and transitions of care
- Educate patients, families, and caregivers on treatment plans and health management
- Contribute to development of clinical policies, procedures, and care standards
- Participate in staff education, training, and IDT meetings
- Maintain accurate, compliant clinical documentation
- Ensure adherence to all regulatory and organizational policies
Practice Setting
- Patient Population: Geriatric patients in a PACE (Program of All-Inclusive Care for the Elderly) setting
