  • Physician ABEM or ABOEM or Board Eligible
  • Flu/COVID/TB NIHSS Stroke Certification,
  • 16 hrs of Stroke CMEs per year
  • Procedure logs: last 24 Months 10 Years of Claims/Coverage History
  • CV, driver’s license, SS card, color photo, state medical license (including licensure held in other states Peer Refs are defined as physician to physician; DPM to DPM; DDS to DDS who have knowledge
