Southern California Transitional Care Internal Medicine Job

• Seeking a board certified Internal Medicine Physician for a newly created program in Transitional Care
• Outpatient primary care clinic that will begin to implement Transitional Care for their patients for acute care
• & chronic conditions
• Must have at least three years’ experience
• Administration/Director/Leadership Experience is a plus
• Permanent job opening
• Full time only: Monday – Friday, 8am-5pm
• Work 5 hours per month in the evening or weekend (provider will be compensated)
• Work with anywhere from 5-10 physicians/NPs
• Supervision of APPs
• Call: evening phone only (2 wks per year)
• See about 12-14 patients per day
• BLS and first aid required
• H1 and J1 visa candidates welcome to apply
• EMR: EPIC

• Multiple office locations in Los Angeles County
