- This is a permanent position
- We are seeking a physician who is board certified in internal medicine (IM) or family medicine (FM) with a passion to serve older adults, who enjoys working with patients in all levels of post-acute care including LTAC, sub-acute rehab, and inpatient rehab.
- Responsibilities:
- Work alongside pharmacists on medication management
- Complete Medicare required visits
- Complete acute, follow up, and discharge visits as necessary
- Implement an individualized plan of care for patients' goals
- In collaboration with the IDT determine transitional care
- Make recommendations on DME, home health services, or outpatient services after short stay
