Nursing Home Nurse Practitioner

Position Overview

A large multispecialty group in NYC is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to provide high-quality care in long-term care and subacute nursing facilities. This role is ideal for experienced providers interested in a supportive, patient-focused environment.

The provider will conduct routine primary care and follow-up visits, manage chronic conditions, and collaborate with facility staff and supervising physicians. 

H1B sponsorship available.

Key Responsibilities

• Perform comprehensive physical exams, patient assessments, and follow-up visits

• Diagnose and manage a variety of medical conditions in the long-term care setting

• Order and interpret diagnostic tests, labs, and imaging studies

• Administer and prescribe medications and treatments according to protocols

• Vaccinate and educate patients on preventive care, hygiene, and diet

• Maintain accurate patient records in the EMR (eClinicalWorks), including medical history, medications, allergies, assessment, and plan

• Collaborate with facility staff, multidisciplinary care teams, and subspecialists within the network

• Ensure high-quality care delivery and compliance with organizational standards

• Participate in ongoing professional development and quality improvement initiatives

Qualifications

• Master’s degree from an accredited NP or PA program

• Current NP or PA license in New York State

• NCCPA (for PAs) or relevant certification for NPs

• Experience in nursing home, long-term care, or geriatric specialty strongly preferred

• Strong clinical, diagnostic, and critical thinking skills

• Excellent communication, interpersonal, and teamwork abilities

• Ability to work independently in a fast-paced care setting

Schedule & Flexibility

• Full-time or part-time options available

• Multiple boroughs available – choose Bronx or Manhattan based on preference

Compensation & Benefits

• Competitive salary: $135,000 – $150,000 annually
