Nurse Practitioner (NP) / Physician Assistant (PA) – Farmington, NM

Join our team as an Advanced Practice Clinician (APC) providing high-quality care in a post-acute setting in Farmington, New Mexico.

Position Highlights

• Full-time role with flexible, self-directed scheduling

• 15–20 patients per day

• Day call (Monday–Friday) with rotating weekends

• Compensation: $132,000–$157,000 + uncapped productivity bonuses

• Benefits eligible + signing bonus and relocation assistance

Responsibilities

• Deliver safe, cost-effective care in skilled nursing and long-term care settings

• Collaborate with interdisciplinary teams and follow CMS guidelines

• Coordinate with hospice, palliative, and home care providers

• Ensure timely documentation and billing (Gehrimed EMR)

Qualifications

• Board Certified NP (FNP, AGACNP, ACNP, AGPCNP) or PA-C

• Active state license and DEA

• Experience preferred; training available
