Seeking a
Nurse Practitioner or Physician Assistant with a Master’s degree from an accredited Program


Two plus years’ experience as a Nurse Practitioner or Physician’s Assistant highly desirable; minimum of two years of experience in a community health
  • Active Nurse Practitioner or Physician Assistant License in the state of California;
  • DEA License;
  • Current BLS certificate
  • Active NPI

Fridays and Saturdays Weekly 8-20 hours

Responsible for the delivery of patient-centered, team-based care that falls within the allowable scope of practice. This includes working collaboratively with physicians, registered nurses and medical assistants to provide advanced practice physical and psychosocial assessments, diagnosis and management of health and illness needs.  

The primary responsibility of the Nurse Practitioner or Physician Assistant is direct patient care for all ages and genders, including both acute and chronic care.  

The Nurse Practitioner or Physician Assistant will also assist the Chief Medical Officer, Medical Director, and Staff Physicians in supervision of the nursing staff.  
  • Provide high-quality, patient-centered and values-based primary medical care.
  • Effectively use huddles, care gap reports, and integration work to meet treatment and preventative care measures.
  • Provide direct education to patients regarding their medical conditions and treatment options.
  • Appropriate and complete referrals to specialty care as needed.
  • Prescribe and/or dispense medications as appropriate.
  • Efficiently and timely manage and respond to requests for medication refills, document completion, patient messages and general questions.
  • Collaborate with behavioral health, dental, optometry and other health care team members as appropriate for integration, consultation, referral and follow-up.
  • Assist the Chief Medical Officer and Medical Director in program development, implementation and evaluation as necessary.
  • Active participation in the health center’s quality assurance and peer-review programs.
  • Provide primary medical care services in accordance with health center policies and board approved privileging.
  • Provide coverage to the health center’s urgent care schedules.  
  • Practice in an efficient and cost effective manner; appropriately order labs and tests.
  • Provide on-call coverage for after-hours care in accordance with health center protocols.
  • Lead the care team in problem-solving to see late arriving patients and patients with urgent needs.
  • Collaborate with the medical management team to maximize patient appointment access.
