Availability Needed (Block Schedule)

Onsite Orientation Required: April 1 (approximately 8am – 3pm)

Coverage Blocks:

• April 2–8: Calumet
• April 8–14: Houghton
• April 16–22: Calumet
• April 23–29: Houghton
• April 30 – May 6: Calumet
• May 7–13: Houghton
• May 14–20: Calumet
• May 21–27: Houghton
• May 28 – June 3: Calumet

Additional ongoing coverage needs expected.

Clinical Details

• Specialty: Express Care NP or PA
• Setting: Solo provider
• Patients per Day: 14–28
• Schedule: 7 days per week
• Support Staff: 1 MA, 1 receptionist
• Trauma Level: III

Required Licenses

• Active Michigan (MI) License
• Active Michigan Controlled Substance (CS) License
• Active DEA (MI)

Required Certifications and Credentials

• Board Certified NP or PA (required)
• BLS Certification (required)
• US Board Certified
• Clinical experience within the last 12 months
  OR completion of an accredited NP/PA program within the last 12 months
