Sacramento, California – Nephrology Nurse Practitioner Job (Post-Transplant Care)

- Seeking a Nurse Practitioner with experience in nephrology, transplant, or complex chronic disease management
- Full-time position: Monday–Friday, 8:00 AM – 5:00 PM PST
- Field-based role (clinic, in-home visits, and occasional telehealth)
- Hiring due to practice growth
Clinical Responsibilities:
- Manage kidney transplant recipients 1+ year post-transplant
- Focus on chronic disease management (hypertension, diabetes, CKD, cardiovascular risk)
- Review labs, monitor transplant function, and adjust medications within scope
- Coordinate care, referrals, and follow-up plans
- Educate patients on medications, lifestyle, and long-term care
- Participate in interdisciplinary team (IDT) meetings and quality initiatives
- Perform Annual Wellness Visits and comprehensive health evaluations
- Conduct in-home visits within the local region
Requirements:
- Active California NP license and DEA (or willing to obtain)
- AANP or ANCC certification; BLS required
- Minimum 2 years of relevant experience (transplant, nephrology, or complex chronic care)
- Knowledge of immunosuppressants and transplant protocols preferred
- Valid California driver’s license required
- Spanish bilingual skills preferred
- EMR/transplant workflow experience a plus

About the Position & Benefits:
