Critical Requirements (All Required at Submission)

• Board Certified in Hematology – REQUIRED
• Active State License – REQUIRED
• Completed Residency/Fellowship – REQUIRED
• DEA – REQUIRED
• BLS – REQUIRED
• EPIC EMR experience – REQUIRED

Preferred

• ACLS certification

Availability

• Start: ASAP
• Duration: 6–12 months

Schedule

• Full-time, Monday–Friday
• 8:00 AM – 5:00 PM
• No call

---

Practice Setting

• Outpatient only
• Support Staff: MA, RN, Scheduler

Clinical Scope

• Hematology-focused care
• No required procedures

---

Patient Volume

• 20–23 patients per day

---
