Position Details
Start Date: May 15th – Ongoing
Assignment Length: 6 Months
Schedule Options: Choose between (4-5) 10-hour shifts or (5) 8-hour shifts per week
Call: No call required
Employment: Hospital-employed program
Case Mix: Orthopedics, Obstetrics, Vascular, GI, ENT, Cath Lab, and various outpatient procedures
Supervision Model: Anesthesia care team with a 1:4 MD to CRNA ratio
Requirements
Licensure: Active New York State Medical License required
Certifications: ACLS, BLS, NBCRNA certification
Availability: Full-time availability is a must
Experience with EMR: Cerner EMR system
Additional Information
Locum-to-Permanent: Opportunity to transition to a permanent role if desired

Credentialing Requirements
Case logs from the past 2 years.
Certificates of insurance covering the last 10 years.
Employment verification for the past 10 years.
Hospital privileges for the last 10 years.
Three clinical references from the past 6 months (at least one from leadership).
Additional certifications: BLS, ACLS, and Infection Control.
