- Seeking a Nurse Practitioner
- Provide care in long-term care and subacute nursing facilities
- Ideal for experienced providers interested in a supportive, patient-focused environment
- Perform routine primary care and follow-up visits
- Manage chronic medical conditions in the nursing home setting
- Conduct comprehensive physical exams and patient assessments
- Order and interpret diagnostic testing, labs, and imaging studies
- Prescribe medications and treatments according to protocols
- Provide vaccinations and patient education on preventive care, hygiene, and nutrition
- Collaborate with facility staff, supervising physicians, subspecialists, and multidisciplinary teams
- Ensure quality care delivery and compliance with organizational standards
- Participate in ongoing professional development and quality improvement initiatives

- Strong preference for providers with nursing home, long-term care, or geriatric experience
- Must be comfortable working independently in a fast-paced clinical environment

Qualifications:
- Master’s degree from an accredited NP or PA program
- Active New Jersey NP or PA license required
- NCCPA certification required for PAs
- Relevant NP certification required for Nurse Practitioners
- Strong clinical, diagnostic, and critical thinking skills
- Excellent communication and teamwork abilities
Schedule & Flexibility:
- Full-time or part-time options available
