Family Medicine Physician – Santa Cruz County, CA

Clinical Role:

• Permanent position with flexible 4-day work week

• Full-time and part-time options available

• 100% outpatient family medicine with opportunity for dedicated pediatrics time if desired

• See ~20 patients per day

• Team-based medical home model with on-site dental, optometry, behavioral health, lactation support, parent educators, diabetic nurse educator, and MAT program

• EMR: Greenway Intergy

• Due to patient population, must be bilingual in Spanish

• Not able to sponsor visa candidates

Compensation & Benefits:

• Salary: $270,400 – $304,337.58 annually (DOE & FTE)

• Sign-on/Retention Bonus: Up to $30,000 (FTE prorated)

• 50% paid on first paycheck, 25% after year 1, 25% after year 2

• Relocation assistance: Up to $10,000
