Licensure & Certifications

  • North Carolina Medical License – REQUIRED
  • Board Certified – REQUIRED (Internal Medicine, Family Medicine, or Geriatrics)

Experience Requirements

  • Experience Required – MUST have prior clinical experience
  • Leadership or supervisory experience strongly preferred
  • Experience with geriatric populations or complex chronic care highly preferred

Clinical & Leadership Scope

  • Provide medical direction and oversight of participant care
  • Lead Quality Assurance Performance Improvement (QAPI) initiatives
  • Develop and maintain clinical policies, procedures, and protocols
  • Supervise Primary Care Providers (PCPs) and APPs
  • Participate in interdisciplinary team leadership and care planning

Patient Population

  • Seniors 55+ with chronic care needs
  • Nursing home–eligible but living in community setting
  • Focus on maintaining independence and comprehensive care

Key Responsibilities

  • Conduct H&Ps and reassessments
  • Develop and manage treatment plans across care settings
  • Participate in intake and enrollment decisions
  • Lead care planning discussions with patients and families
  • Collaborate across interdisciplinary teams
  • Oversee diagnostic evaluations and clinical documentation
  • Participate in family conferences and care coordination

Practice Model (PACE)

  • All-inclusive, community-based care model
  • Coordinates full continuum of medical and supportive services
  • Emphasis on preventing nursing home placement and maintaining independence
