• Seeking a board certified or board eligible Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP)
• Permanent employed opportunity
• Outpatient primary care only
• Monday–Friday schedule
• Clinic hours: 9:00am–5:00pm
• No call
• No inpatient responsibilities
• Ability to see all ages or adult-only patient panel
• EMR: eClinicalWorks
• Part-time or full-time options available
• Prior primary care experience required
• Preference for candidates who speak Hindi or Urdu (not required)
• Preference for candidates willing to float between offices in New Hyde Park, Glendale, Fresh Meadows, and Forest Hills
• H1B visa sponsorship available
• Responsibilities include preventive care, chronic disease management, acute visits, medication management, health screenings, and patient education
• Diagnose and manage common primary care conditions including diabetes, hypertension, asthma, and hyperlipidemia
• Order and interpret labs and diagnostic imaging
• Refer patients to specialists within the network when appropriate
• Maintain accurate and timely documentation in the electronic medical record
• National Board Certification required (AANP or ANCC)
• Active New York Nurse Practitioner license required
• Bilingual Spanish is a plus
