• Clinical Role:

o Seeking a board certified or truly board eligible family medicine or internal medicine physician
o Outpatient only
o No call
o No weekends
o Part-time or full-time available
o Must be able to work at least 3 days per week
o Average patient volume: 2.5 patients per hour
o Sites provide medical, dental, and behavioral health services
o Work alongside multiple primary care providers
o Physician would supervise APPs
o Multiple openings available – choose the location that best fits your lifestyle: Brockport, Pembroke, Warsaw, or Hornell
o Provider would work at only one location

o Brockport offers a classic college-town atmosphere along the Erie Canal with easy access to Rochester’s amenities, dining, and airport.
o Pembroke provides a quieter rural setting conveniently located between Buffalo and Rochester, with affordable living and a short drive to Batavia.
o Warsaw is known for its scenic rolling hills, outdoor recreation, and close-knit community feel in Wyoming County.
o Hornell, located in the Southern Tier, offers a lower cost of living, beautiful natural surroundings, and access to Finger Lakes recreation and wineries.
o All four communities provide an easy pace of life while remaining within driving distance of larger metro amenities in western New York.

o Competitive salary range of $200,000-$220,000
