Position Overview
Seeking a quality-driven physician to join an established post-acute care team providing comprehensive care in skilled nursing facilities throughout the Hartford area. This opportunity offers strong clinical support, physician autonomy, leadership development, and significant earning potential.
Clinical Responsibilities
- Perform comprehensive patient assessments, including histories and physical exams
- Develop and manage individualized treatment plans
- Monitor patient progress and adjust care plans as needed
- Coordinate care with physicians, nursing staff, and interdisciplinary teams
- Educate patients and families regarding health management and preventive care
- Maintain accurate and compliant medical documentation
