Locums – Geriatric Medicine Physician | Kalispell, Montana (IMLC Accepted)

Availability
- ASAP start
- Ongoing coverage
- Preferred schedule: Tuesday, Wednesday, Thursday
- 10-hour daytime shifts (7:00 AM – 5:00 PM)
- No night call responsibilities
- Beeper call listed; hospitalists cover overnight call
- 30-minute response time if call coverage is assigned
Required Licenses
- Active Montana Medical License OR
- Active IMLC
Required Certifications & Qualifications
- Board Certified or Board Eligible in Internal Medicine or Family Medicine
- BLS Certification
- ACLS Certification
- Experience in Skilled Nursing Facility (SNF), Long-Term Care (LTC), Transitional Care, or Post-Acute Medicine
- Experience caring for geriatric and medically complex patients strongly preferred
Practice Details
- Coverage for Skilled Nursing Facility (SNF) and Long-Term Care (LTC) patients
- Manage both short-term rehabilitation and long-term care residents
- Admit new patients and complete History & Physicals
- Conduct CMS-required physician visits and recertifications
- Manage chronic conditions, preventive care, dementia, frailty, and complex geriatric illnesses
- Evaluate acute changes in condition to reduce ED transfers and hospital readmissions
- Medication management and review of pharmacist recommendations
- Develop and update interdisciplinary care plans
- Participate in care conferences, family meetings, and goals-of-care discussions
- Coordinate transitions between hospital, SNF, LTC, specialty care, and community providers
- Complete all regulatory documentation and compliance requirements

- 99-bed unit
- 12–14 patients per shift
- Admissions required
- Assigned rounding patients
- Trauma Level III
- Dedicated Advanced Practice Provider (APP) support
Ideal Candidate
A Board-Certified or Board-Eligible Internal Medicine or Family Medicine physician with experience in Skilled Nursing, Long-Term Care, Transitional Care, or Post-Acute Medicine who enjoys caring for medically complex geriatric patients. This is an excellent opportunity for a physician seeking a predictable daytime schedule, dedicated APP support, and an ongoing locum assignment with no overnight call responsibilities.
