Availability

• Start: ASAP upon credentialing
• Schedule: Monday–Friday, 8:00 AM–4:30 PM
• No call
• No weekends
• No after-hours
• No telehealth
• Ongoing outpatient coverage
• Availability to provide requested coverage dates required

License Requirements

• Active Illinois Medical License
• Active Illinois DEA
• Active Illinois Controlled Substance Registration (CSR)

Required Certifications & Qualifications

• Board Certified or Board Eligible in Internal Medicine or Surgery
• Wound Care & Hyperbaric Medicine training and experience required
• ACLS (AHA)
• BLS (AHA)
• PALS (AHA)

Submission Requirements

• Board Certified or Board Eligible in Internal Medicine or Surgery – Required
• Wound Care & Hyperbaric Medicine training and experience – Required
• Active Illinois Medical License – Required
• Active Illinois DEA/CSR – Required
• Availability for requested coverage – Required
• Two references per Stratum process – Required
• Clean, easy-to-credential candidates – Highly Preferred

Practice Setting

• Outpatient Wound Care & Hyperbaric Medicine
• 100% Adult & Geriatric patient population
• 12–20 outpatient wound care visits per day
• 1–4 Hyperbaric Oxygen (HBO) treatments per day
• 8 exam rooms
• 2 Hyperbaric chambers

Required Procedures

• Wound evaluations and treatment planning
• Compression therapy
• Wound VAC management
• Hyperbaric Oxygen (HBO) treatment oversight
• Debridements
• Cast applications
• Advanced tissue applications
• Incision and Drainage (I&D)
• Callous paring
• Chemical cautery

Optional Procedures

• Punch biopsies

Support Staff

• Experienced Wound Care RNs
• 1 Advanced Practice Provider in clinic Tuesday–Friday

• EMR: Epic
• 315-bed hospital
• Level II Trauma Center
