Clinical Role:

  • Seeking a board certified Nurse Practitioner (Family or Adult-Gerontology)
  • Permanent, employed position
  • Provide comprehensive home-based primary care throughout the Greater Boston region
  • Care for patients with chronic illnesses, mobility limitations, and complex medical conditions
  • Conduct comprehensive in-home health assessments, physical exams, medication reconciliation, and medical history reviews
  • Diagnose and treat acute and chronic medical conditions
  • Develop individualized treatment plans and prescribe medications as appropriate
  • Coordinate care with primary care physicians, specialists, and multidisciplinary healthcare teams
  • Educate patients and caregivers on disease management, preventive care, medication adherence, and lifestyle modifications
  • Perform follow-up visits and adjust treatment plans based on patient progress
  • Assess urgent medical situations during home visits and coordinate appropriate interventions
  • Maintain timely and accurate electronic medical records
  • Participate in community outreach and health education initiatives as needed
  • Travel throughout the Greater Boston area to provide patient care
  • Flexible scheduling with occasional evenings or weekends based on patient needs

Requirements:

  • Master's degree from an accredited Nurse Practitioner program (Family or Adult-Gerontology)
  • Active Massachusetts Nurse Practitioner license (or eligible)
  • National certification through AANP, ANCC, or ONCC
  • Experience with EMR/EHR systems preferred
  • Spanish bilingual is a plus

Compensation & Benefits:

  • Compensation: $135,000-$200,000 (includes bonus and incentive opportunities)
