Greater Boston, Massachusetts – Nurse Practitioner House Calls Job
Clinical Role:

- Seeking a board certified Nurse Practitioner (Family or Adult-Gerontology)
- Permanent, employed position
- Provide comprehensive home-based primary care throughout the Greater Boston region
- Care for patients with chronic illnesses, mobility limitations, and complex medical conditions
- Conduct comprehensive in-home health assessments, physical exams, medication reconciliation, and medical history reviews
- Diagnose and treat acute and chronic medical conditions
- Develop individualized treatment plans and prescribe medications as appropriate
- Coordinate care with primary care physicians, specialists, and multidisciplinary healthcare teams
- Educate patients and caregivers on disease management, preventive care, medication adherence, and lifestyle modifications
- Perform follow-up visits and adjust treatment plans based on patient progress
- Assess urgent medical situations during home visits and coordinate appropriate interventions
- Maintain timely and accurate electronic medical records
- Participate in community outreach and health education initiatives as needed
- Travel throughout the Greater Boston area to provide patient care
- Flexible scheduling with occasional evenings or weekends based on patient needs

Requirements:
- Master's degree from an accredited Nurse Practitioner program (Family or Adult-Gerontology)
- Active Massachusetts Nurse Practitioner license (or eligible)
- National certification through AANP, ANCC, or ONCC
- Experience with EMR/EHR systems preferred
- Spanish bilingual is a plus

Compensation & Benefits:
- Compensation: $135,000-$200,000 (includes bonus and incentive opportunities)
