- Philadelphia County
- Delaware County
- Montgomery County
- Chester County
- On-Call Schedule: Participates in provider call rotation approximately two 1-week assignments every 3–4 months
Practice Setting
- Lead medical oversight for a comprehensive senior care program serving 11 locations throughout the Philadelphia area
- Role includes both clinical care and administrative leadership responsibilities
- Supervise Primary Care Providers and Nurse Case Managers
- Provide leadership for Quality Assurance Performance Improvement (QAPI) initiatives
- Collaborate with interdisciplinary teams to deliver patient-centered care for frail and elderly participants
- Regular involvement with:
- Participant assessments and treatment planning
- Nursing home visits
- Center-based participant care
- Advance care planning discussions
- Care coordination across healthcare settings
- Emergency medical interventions
- EMR optimization and documentation standards
- Medical policy and protocol development
Required Licenses
- Active Pennsylvania physician license (Required)
- CDS Certification (Required)
- DEA Registration (Required)
Required Certifications
- Board Certified in Internal Medicine or Family Medicine (Required)
- Board Certification in Geriatrics (Preferred)
- BLS Certification (Required)
Required Qualifications
- Minimum 1 year of experience working with frail or elderly patient populations (Required)
- Previous supervisory experience (Required)
- Strong communication, leadership, and organizational skills
- Ability to collaborate with multidisciplinary teams and diverse patient populations
- Proficiency with Microsoft Office Suite
- Ability to adapt to changing priorities
- Ability to travel within the program’s geographic region as needed
