Geriatric Medicine Medical Director Opportunity – Philadelphia, Pennsylvania

Availability

  • Position Type: Full-Time Permanent
  • Role: Medical Director

  • Travel Requirements: Local travel required across program sites in:
    • Philadelphia County
    • Delaware County
    • Montgomery County
    • Chester County
  • On-Call Schedule: Participates in provider call rotation approximately two 1-week assignments every 3–4 months

Practice Setting

  • Lead medical oversight for a comprehensive senior care program serving 11 locations throughout the Philadelphia area
  • Role includes both clinical care and administrative leadership responsibilities
  • Supervise Primary Care Providers and Nurse Case Managers
  • Provide leadership for Quality Assurance Performance Improvement (QAPI) initiatives
  • Collaborate with interdisciplinary teams to deliver patient-centered care for frail and elderly participants
  • Regular involvement with:
    • Participant assessments and treatment planning
    • Nursing home visits
    • Center-based participant care
    • Advance care planning discussions
    • Care coordination across healthcare settings
    • Emergency medical interventions
    • EMR optimization and documentation standards
    • Medical policy and protocol development

Required Licenses

  • Active Pennsylvania physician license (Required)
  • CDS Certification (Required)
  • DEA Registration (Required)

Required Certifications

  • Board Certified in Internal Medicine or Family Medicine (Required)
  • Board Certification in Geriatrics (Preferred)
  • BLS Certification (Required)

Required Qualifications

  • Minimum 1 year of experience working with frail or elderly patient populations (Required)
  • Previous supervisory experience (Required)
  • Strong communication, leadership, and organizational skills
  • Ability to collaborate with multidisciplinary teams and diverse patient populations
  • Proficiency with Microsoft Office Suite
  • Ability to adapt to changing priorities
  • Ability to travel within the program’s geographic region as needed
