We are seeking a Board-Certified Primary Care Physician in Internal Medicine or Family Medicine to join an established, senior-focused outpatient primary care practice in Port St. Lucie, Florida.
This is an excellent opportunity to join an experienced care team with an established patient panel and a unique value-based care model focused on quality outcomes, preventative care, and spending meaningful time with patients.
Practice Highlights
Outpatient Only
Monday–Friday, 8:00 AM–5:00 PM
Onsite, in-person position
Established patient panel
Senior-focused Medicare Advantage population
Average daily census of 14–17 patients
45-minute authorized appointments, allowing more time with patients
Focus on quality patient care rather than high-volume/RVU-driven medicine
Collaborative care team including Physicians, Nurse Practitioners and/or Physician Assistants, Medical Assistants, and Care Coaches
Opportunity to address both medical needs and social determinants of health
Value-based care model with a strong emphasis on preventative care and quality outcomes
Athena EMR
DAX AI documentation software available
Qualifications
Board Certified in Internal Medicine or Family Medicine
Minimum of 2+ years of primary care experience
Experience managing a Medicare Advantage patient panel preferred
Knowledge of Medicare guidelines and coverage
Knowledge of HEDIS quality indicators
Experience in outpatient primary care, private practice, or a similar clinical environment
Knowledge of or experience with value-based care
Active Florida medical license (ability to obtain prior to start)
Spanish and English fluency preferred
