SCHEDULE & AVAILABILITY
- Permanent, full-time position
- Monday through Friday
- 8:00 AM–5:00 PM
- 100% outpatient practice
- No inpatient responsibilities
- Phone/beeper call approximately 1 week annually
POSITION OVERVIEW
Seeking a Board-Certified Family Medicine Physician for an outpatient community health practice serving an underserved patient population.
The physician will provide comprehensive primary care to patients across all age groups in a collaborative, patient-centered environment.
PATIENT MIX & CLINICAL VOLUME
- Approximately 20 patients per day
- All-age patient population
- Approximately 30% pediatric patients
- Approximately 70% adult patients
- Approximately 1.5 days per week dedicated to Pediatrics
- Approximately 3.5 days per week dedicated to Adult Medicine
CLINICAL RESPONSIBILITIES
- Provide comprehensive outpatient primary care for pediatric, adolescent, adult, and geriatric patients
- Diagnose, treat, and manage acute and chronic medical conditions
- Conduct preventive health screenings and routine physical examinations
- Provide wellness visits and preventive care
- Manage chronic diseases and develop individualized treatment plans
- Provide pediatric primary care, including well-child visits, immunizations, and treatment of common pediatric illnesses
- Educate patients and families regarding disease prevention, health maintenance, and treatment plans
- Coordinate care with specialists, ancillary services, and interdisciplinary teams
- Maintain accurate and compliant documentation within Epic EMR
- Participate in limited annual phone/beeper call coverage
- Support quality improvement and patient-outcome initiatives
- Provide culturally sensitive, patient-centered care to an underserved community

- Ability to effectively manage both pediatric and adult patient populations
- Experience serving underserved or community-based populations preferred
- Excellent diagnostic, clinical, and decision-making skills
- Strong communication and patient education skills
- Ability to establish trusting relationships with patients and families
- Experience working within multidisciplinary healthcare teams
- EMR proficiency, preferably Epic
- Strong organizational, documentation, and time-management skills
