SCHEDULE & AVAILABILITY
- Permanent, full-time position
- Monday through Friday
- 8:00 AM–5:00 PM
- 100% outpatient
- No inpatient responsibilities
- Phone/beeper call approximately 1 week annually
POSITION OVERVIEW
Seeking a Board-Certified Family Medicine or Internal Medicine Physician for a growing outpatient practice serving an underserved adult patient population.
This hybrid primary care role provides comprehensive medical care to multiple patient groups, including:
- Patients requiring acute response care
- Post-discharge patients requiring transitional care management
- Patients referred from specialty clinics who require primary care services
PATIENT VOLUME & PRACTICE
- Approximately 20 patients per day
- Adult patient population
- Underserved and medically complex patient population
- Epic EMR
- Collaborative, patient-centered environment
- Opportunity to support population health and continuity of care
CLINICAL RESPONSIBILITIES
- Provide comprehensive outpatient primary care for adult patients
- Evaluate, diagnose, and treat acute and chronic medical conditions
- Provide transitional care for patients following hospital discharge
- Provide primary care services for patients referred from specialty clinics
- Manage preventive health screenings and chronic disease care
- Develop and manage individualized patient care plans
- Coordinate care with specialists, care managers, and interdisciplinary healthcare teams
- Maintain accurate, timely, and compliant documentation within Epic EMR
- Participate in limited phone/beeper call coverage
- Support quality improvement initiatives focused on patient outcomes and access to care
- Maintain compliance with organizational, regulatory, and accreditation standards
- Provide compassionate and culturally competent care to an underserved population
SKILLS & EXPERIENCE
- Strong outpatient primary care experience with adult patients
- Ability to effectively manage acute and chronic medical conditions
- Experience caring for medically complex and underserved populations preferred
- Excellent clinical judgment and diagnostic skills
- Strong communication and patient education abilities
- Ability to collaborate effectively within a multidisciplinary healthcare team
- Proficiency with electronic medical records, preferably Epic
- Strong organizational and time-management skills
