Nurse Practitioner (NP) / Physician Assistant (PA) – House Calls

Position Overview

A large multispecialty group in New York City is seeking Nurse Practitioners (NPs) and Physician Assistants (PAs) to provide high-quality, patient-centered care in patients’ homes and nursing facilities. This role is ideal for providers who enjoy holistic care, chronic disease management, and direct patient engagement.

New graduates are welcome to apply! This is an excellent opportunity for newly licensed NPs and PAs to join a supportive multispecialty organization with robust provider infrastructure, EMR training, and ongoing professional development.

Providers have the option to work in Brooklyn, Manhattan, or The Bronx, offering flexibility based on geographic preference.

Key Responsibilities

Patient Care & Evaluation

• Conduct comprehensive patient assessments, including physical exams, medication reconciliation, and medical history review during home or nursing facility visits

• Diagnose medical conditions and develop individualized treatment plans

• Prescribe medications and manage acute and chronic illnesses within scope of practice

Care Coordination & Collaboration

• Collaborate with physicians, specialists, and multidisciplinary teams to ensure continuity of care

• Provide referrals and coordinate follow-up with other healthcare providers

• Communicate patient needs and clinical updates with the care team

Patient & Family Education

• Educate patients and families on disease management, lifestyle modifications, medication adherence, and preventive care

• Answer patient and family questions and provide guidance regarding treatment plans

Documentation & Follow-Up

• Maintain accurate and timely documentation in the EMR/EHR

• Schedule and conduct follow-up visits to monitor patient progress and adjust treatment plans as appropriate

Emergency Response & Community Engagement

• Assess and manage urgent medical issues during visits

• Participate in community outreach and patient education initiatives

Qualifications

Nurse Practitioner

• Master’s Degree from an accredited NP program

• Family, Adult-Gerontology, or related NP focus preferred

• Current New York State NP license

• National Board Certification through AANP, ANCC, or applicable certifying organization

• New graduates are welcome to apply

Physician Assistant

• Master’s Degree from an accredited Physician Assistant program

• Current New York State Physician Assistant license

• NCCPA certification

• New graduates are welcome to apply

For All Providers

• Due to the patient population, the client is seeking providers who are bilingual in Russian or Chinese

• Experience with EMR/EHR systems preferred

• Strong clinical judgment and communication skills

• Ability to work independently while collaborating effectively with physicians and multidisciplinary teams

• Willingness to provide care in patients’ homes and nursing facilities

Compensation & Benefits

• Competitive salary: $135,000 – $150,000 annually
