LOCUMS OPPORTUNITY – NURSE PRACTITIONER / PHYSICIAN ASSISTANT

POSITION OVERVIEW

• Provider Type: Nurse Practitioner (NP) or Physician Assistant (PA)
• Start Date: November 30
• Schedule: Monday–Friday, 8:00 AM–5:00 PM
• Locations: Surprise, Litchfield Park, and Chandler, Arizona
• Practice Setting: Outpatient Only
• Care Model: Value-based primary care
• Patient Population: Ages 0–65; all ages preferred, not required
• Language: Bilingual preferred, not required
• EMR: eClinicalWorks

AVAILABILITY & SCHEDULE

• Monday–Friday, 8:00 AM–5:00 PM
• No call
• No weekends
• No inpatient responsibilities
• Start date: November 30

LICENSES & CERTIFICATIONS

• Arizona NP or PA licensure requirements to be confirmed
• NPs: Board certified through AANP or ANCC (Family NP preferred)
• PAs: NCCPA certified
• Active DEA registration

CLINICAL REQUIREMENTS

• Post-training clinical experience required; new graduates will not be considered.
• Strong clinical background in outpatient primary care.
• Value-based care experience required, including quality measures, care gap closure, preventive care, and chronic disease management.
• Must be comfortable treating patients of all ages.
• Must be comfortable performing annual women’s wellness exams.
• Experience with eClinicalWorks preferred.

• Brand-new clinic locations across the Phoenix area.
